PROCEDURES FOR APPLYING FOR CARIBBEAN COMMUNITY GRENADA PASSPORT
PLEASE INSERT SIGNATURE IN THE MIDDLE OF BOX ON UPPER RIGHT-
HAND CORNER OF FIRST PAGE OF APPLICATION
1. All application MUST be handwritten, using a pen and in block capital letters
2. All applicants MUST complete sections 1,3, 4, 6, 8, 10, 11, 12

Applicants who are married/divorced must also complete Section 2 (please submit divorced or marriage certificate for name changes).

Applicants born outside of Grenada must also complete Section 5 and submit their Grenadian parent original birth certificate.

Applicants Replacing Passports (Lost, Stolen) must complete Section 7.
3. Section 12: Recommender MUST be completed by a Grenadian (not a relative) who knows the applicant for at least 3 years.
4. Children under the age of 16 years must have written consent of their parent or legal guardian when applying for passport. (section 9 must be completed).
5. All applicants MUST produce their original Birth Certificates, or Certificates of Naturalization or Registration and previous passports with application form.

For lost passport applications, a Police Report and Affidavit must be submitted. (the affidavit form can be obtained from the Consulate or can be down loaded from our website www.grenadaconsulate.org).

Applicants for lost passport must obtain a letter from the Consulate in order to obtain a precinct report/incident slip
6. Two recent passport size pictures (full face showing both ears no head covering e.g. hats etc.) must be submitted with the application.
Recommender (in section 12) MUST endorse the reverse side of one photo with the words: “I certify that this is a true likeness of the applicant Mr. (Mrs. or Miss)…………..” and add his signature.
